Primary Care Membership
When you're sick, you're sick. Getting an appointment quickly for your urgent care needs gets you back in the game of life. As a member of Fort Family Health, same day appointments are our priority and guarantee at least next day appointments. If we can safely treat your illness or needs over the phone or virtually, Jennifer is willing to do that for you in times of urgency. Late evening appointments are available on Tuesdays until 8 pm to help alleviate the burden of having to miss school or work for an appointment also.

- Single Adult Membership <55yrs $599.00/year
- Single Adult Membership >55yrs $699.00/year
- Family Membership (from 2 – 6 individuals) <55yrs * $999.00/year (additional members will be $100.00 each)
- Family Membership (from 2 – 6 individuals) >55yrs * $1099.00/year (additional members will be $100.00 each)
*Family membership is defined as a married couple, a set of parents, or a single parent with dependent children under the age of 18 years old.
